Major, major issues with power generation, angling, and more. You're getting too caught up in overthinking things, trying to force things into a shape they don't match. Additionally, there's actually quite a gap in reality with the attacker there. I can see how you're doing a form of your Sword and Hammer, so you're keeping consistency there, but there's still some large issues.
 
I practice and teach both Kenpo and Judo... Good attempt but as the poster above me stated it appears you are over thinking too much. I also agree with his (Chris Parker) comment in reference to loss of power and misuse of angles... It appears that you are striking to areas that would have little to no effect in a real fight (the legs for example). A real attacker would flail his legs (while on his back) quite a bit more as well... Just what my training as shown me
